2024 Employee of the Year Nomination Form
WHO IS ELIGIBLE:

WHO CAN NOMINATE:
Any person can submit nominations for someone they believe makes a positive impact in their work.
HOW WINNER IS CHOSEN:
Nominations are reviewed by the Chamber Board of Directors and a winner is selected based on the CONTENT of the submitted nomination. PLEASE PROVIDE DETAILS to support your nomination. Please note that your submission, if chosen, may be used in a press release as well as read directly during the program.
DEADLINE TO NOMINATE:
The nomination form is due Monday, November 04, 2024. Late entries not accepted.
AWARD PRESENTATION:
*NOMINEE ATTENDANCE IS REQUIRED* at the award ceremony on November 20, 2024 from 11:30am-1:00pm. All nominees will be recognized and one nominee selected as Employee of the Year.
